AI will help shape their world. We should help shape AI.
Every child, parent, patient, worker, and community deserves AI that earns trust through evidence, accountability, and respect for people.
See why this mattersNot fear. Not blind faith. Proof, boundaries, and human dignity.
Learning
AI can shape how people learn and how children are evaluated.People deserve clear notice, age-appropriate safeguards, and a reachable human when an automated system affects learning.Explore every life momentCloser than it feels
AI reaches into the moments that make a life.
The conversation is not only about technology. It is about the people affected by decisions made with it, around it, and sometimes because of it.
Health
When care depends on a recommendation
Patients deserve systems that are examined for evidence, limitations, privacy, and the consequences of being wrong.Opportunity
When a person applies for work, housing, or credit
People deserve meaningful review, understandable decisions, and a real path to challenge harmful outcomes.Home
When technology enters the most private parts of life
Consent should remain real, personal information should remain protected, and convenience should never erase dignity.Information
When a system influences what someone believes
Everyone deserves transparency about automated content, clear limits, and safeguards against manipulation.Safety
When software becomes part of a critical decision
The greater the consequence, the stronger the evidence and independent oversight should be.The questions behind the worry
Families are not asking for perfection.
They are asking whether anyone responsible has truly looked.
01“Who checked the system before it reached my family?”
02“What happens when the AI is confident and wrong?”
03“Can a person say no, appeal, or ask for a human?”
04“Who protects the information it learns about us?”
05“Will anyone notice when the system changes?”
06“Are we being asked to trust a promise, or shown proof?”

Share your experience through a controlled public-interest intake. Your story may help identify where stronger transparency, human review, and independent examination are needed.
Share Your AI ExperienceNo one should feel alone
A place for voices, when people choose to share them.
When separate public-sharing permission is available and a short excerpt is reviewed and approved, it can appear here as Anonymous or with their chosen first name and broad region. A report is never a verified finding.
The globe counts distinct incidents, patterns, hazards, and regulatory matters from original records and attributed public reports. A source-linked report is not an independently verified finding. AI systems and other automated decisions are identified separately. These records are separate from experiences shared through this site and are not a count of every incident worldwide.
For broader reported cases, explore the OECD AI Incidents and Hazards Monitor and the AI Incident Database. These catalogs are starting points for research; their entries are not independent findings by Clause5afe or proof of a legal violation.
There is room for your voice here.
No experiences have been approved for public display. Reports are private by default; a public excerpt or name needs a separate choice and review.
Share your experienceExplore the documented record
84 records have a country on the globe; 6 global records are listed without a geographic pin. 33 matter or case records use an original source; 57 are attributed source-linked reports. These are editorial research records, not private submissions or proven regulatory violations.
Drag to spin on desktop; swipe on a phone or tablet. Use a mouse wheel or trackpad over the globe to zoom, or use the +, −, and reset controls. At closer zoom, numbered state badges show state-specific records; hover, tap, or choose a region for details. The directory below includes locations on the far side of the globe. Counts change with the filter and use only broad country or U.S. state locations. A blank region does not mean no incident occurred there.

Retail facial recognition falsely flagged customers
The FTC charged that Rite Aid's AI-based facial recognition produced false matches in stores and that employees confronted or removed customers based on those alerts. The case ended in a stipulated order restricting Rite Aid's use of the technology.
FTC allegations were resolved by an order; the map does not imply that every alleged event was independently adjudicated.Read the Federal Trade Commission sourceAI-generated hiring advertisements and citizenship restrictions
The Justice Department announced a settlement concerning job advertisements generated by an AI tool that included citizenship restrictions not authorized by law.
The settlement addresses the department's allegations; it is not a separate finding by Clause5afe.Read the original U.S. Department of Justice recordTrust should be earned
Safety needs more than a promise.
No technology can be made entirely risk-free. But high-consequence systems can be held to stronger, clearer, independently examined expectations.
Evidence before reassurance
Trust should begin with documented testing, defined scope, and a clear account of what was and was not examined.
Independent eyes
The organization building or selling an AI system should not be the only organization deciding whether its claims are credible.
Human accountability
There should always be people responsible for the system, its limits, its decisions, and the response when something goes wrong.
Protection that continues
AI changes. Its data, model, use, and environment can change too. Trust must be revisited when the system materially changes.
A public expectation
Five things every person should be able to ask of AI.
Simple questions can create stronger expectations for companies, institutions, and public systems.
- 01
Tell people when AI is meaningfully involved.
- 02
Protect privacy, consent, and the right to human review.
- 03
Name the system's limits as clearly as its capabilities.
- 04
Use independent evidence for high-consequence claims.
- 05
Keep a visible record when trust status changes.
Your experience can help
Help shape what trustworthy AI must become.
The systems affecting people are easier to understand when people can describe what actually happened. You may submit anonymously, and every optional use of your story has a separate choice.
Our promise to you
Clause5afe will never sell your data. Ever.
Your experience belongs to you. Anonymous reporting remains available, and contact, research, outreach, quoted-use discussion, email, and text permissions remain separate choices you control.
Review every intake and privacy choiceThe future is still ours to shape
We do not have to choose between progress and people.
We can welcome what AI makes possible while expecting evidence, limits, accountability, and respect for the humans living with its consequences.
